They are generally agreed upon by either the W3C (World Wide Web Consortium), or more recently by the WHATWG (Web Hypertext Application Technology Working Group). These are organizations of several members who find it mutually beneficial to develop standard means of communication over the internet. After a standard has been approved, or even just recommended, the web browsers will try to include this functionality into them. At that point, web designers can start to use these features in their web pages.

It depends on the standards that you are talking about. If you are speaking strictly “web” then the World Wide Web Consortium (http://www.w3.org/) reviews and approves them. If you are talking about standards for different protocols, they are typically put into request for comment (RFC (http://www.faqs.org/rfcs/)) form and evaluated as open source.
